Cardiologist Embedded in Primary Care Team Seeking a board-certified or board-eligible Cardiologist to join our primary care team, focusing on coordinated and integrated patient care within a value-based model. This innovative role is designed to improve patient outcomes, enhance access to cardiology, ensure adherence to clinical guidelines, and reduce unnecessary referrals to specialty care.Essential Functions

• Assess and monitor the medical needs of patients and significant others.
• Provide quality, cost-effective medical services to patients, ensuring consistent coverage of caseload.
• Offer input in policy and procedure development related to medical practice and quality patient care.
• Oversee Physician Assistants, Nurse Practitioners, and other delegated care providers.
• Evaluate educational needs of patients, staff, and significant others, implementing or delegating appropriate teaching strategies.
