Were looking for an organised and proactiveCare Coordinator to support the delivery of high-quality, person-centred care while helping to grow and develop our services.

This is a fast-paced role requiring strong communication, rota management, and the ability to build relationships with clients, staff, and healthcare professionals.

• Coordinate care packages from referral through to delivery
• Manage and optimise staff rotas (holidays, absence, coverage)
• Recruit, onboard, and support care staff
• Liaise with clients, carers, and professionals (GPs, Commissioners, Nurses)
