About the role
We are currently recruiting for a 0.9 FTE (9 days/fortnight) 12-month fixed term Care Partner based in Bacchus Marsh with travel to main office in Ballarat once a month.

As a Care Partner, you are at the heart of our mission, empowering older adults to remain independent, safe and connected within their own homes and communities. Your days will be filled with meaningful interactions, as you:
Deliver high-quality, person‑centred care to clients in their homes and communities

Conduct comprehensive assessments, develop care plans and coordinate services

Empower clients and carers to make informed choices about their care

Ensure compliance with aged care quality standards and organisational policies

Maintain accurate case notes, documentation and client confidentiality

Work collaboratively within a supportive, multidisciplinary team and escalate concerns where required
