An exciting opportunity has arisen for a clinical coordinator to join our Community Rehabilitation Team (CRT) based Glenrothes Hospital. CRT has the overall aim of improving the quality of care and outcomes for older people with the intention of allowing them to remain independent within their own homes or communities. A key aspect of the role is supporting the patient flow through the health system by facilitating early supported discharges from the acute hospitals and by preventing admission to hospital. This will be done by providing therapy and nursing interventions to patients on the caseload. This post is essential part of the multidisciplinary team which also includes physiotherapists, occupational therapists, nurses and healthcare support workers.
