Job overview We are looking for a Senior Nurse, experienced working with patients in the community with complex long‑term conditions. 
You will join with our North Kerrier Community Matrons who, as part of the wider Community Nursing Team, work closely across Adult Community Services, Adult Social Care and Primary Care, using enhanced clinical skills to enable patients to remain independent at home for as long as possible and with the aim of reducing the risk of avoidable hospital admissions.
