Description and Responsibilities
Open Sky’s Community Supports Program is a Mass Health funded navigator service that provides case management and care coordination to individuals in the community. Individuals served in the program must have a mental health disorder or co-occurring disorder and be considered at high risk for admission to a 24-hour treatment setting.
The Community S ervices Navigator will provide case management and direct outreach support to their assigned caseloads , which consist of members enrolled in Community S ervices Program s , Recovery Support Navigation, Behavioral Health Justice Involved and/or C ommunity Support Program for People Experiencing Chronic Homelessness.
Other Key Responsibilities:
+ Develop a Support Plan with the member that guides services in a way that is consistent and supportive of other existing comprehensive care plans. Complete all Assessments and Support Plans according to prescribed...
