To support the mission of Helio Health, theContinuing Care Coordinatorwill provide rehabilitation services to patients as a part of their overall treatment. TheContinuing Care Coordinatorwill work on aFull-Timebasis with ourMeadows CCBHCOpen Access Program.
The schedule will beMonday through Friday, 7:30am-4:00pm.
In addition to Helio Health’s comprehensive benefits package, Full-Time hires for this position are eligible for aSign-On Incentiveup to$1,500!
Pay Rate:$20.00 - $25.00 per hour.
