Job Summary: The Physical Therapist provides comprehensive evaluation, treatment planning, and ongoing therapy services for program participants aged 55 and older. This includes acute, restorative, and maintenance-level care. The PT integrates therapy plans into the physician-directed Plan of Care (POC), collaborates closely with an interdisciplinary team, and ensures all care aligns with InnovAge policies, CMS guidelines, and state regulations.
