Role
This role seeks to support the Future Care Planning Resource Centre and the development of the ecosystem of support for future care planning (FCP). The role involves driving initiatives to meet the FCPRC’s goals.
Responsibilities- Facilitate Future Care Plans Development
- Triage referrals on Future Care Planning.
- Support the management of complex cases surfaced by Care Planners on the ground, through the development of FCP domain resources and network of subject matter experts.
- Facilitate development of future care plan cases unknown to SSAs.
- FCP Sector Capability Development
- Work jointly with the OIC of FCP sector capability development to address capability gaps and create tailored training solutions for FCP professionals.
- Mentor sector professionals and Care Planners in the delivery of FCP.
