Travel between 4 skilled nursing facilities during scheduled shift
Schedule:
Full-Time 40 hours/week
Monday Friday 8:00 AM 5:00 PM
No nights. No weekends. No holidays. No on-call.

Senior Community Care (SCC) program is a longitudinal, integrated care model designed to provide proactive, coordinated care to some of our highest-risk members residing in facility settings.
