Locality Pharmacy Lead

Main duties of the job

The post holder leads on long-term condition management, complex polypharmacy, and promotes health equity by ensuring timely access to medicines expertise. They support GP practices within the locality, work as part of a multidisciplinary team across sites, and ensure provision of clinical support to care homes.

The role has oversight of medicines optimisation and management, including reporting, responding to safety alerts, and supporting digital systems such as SystmOne, Ardens and AI tools. It contributes to clinical governance by supporting investigations, quality improvement, and escalation of risks.

In addition, the role includes clinical line management, appraisals, and workforce development of the Senior Pharmacists, ensuring alignment with organisational values.
