Nurse Practitioner

Pride Health is hiring a Nurse Practitioner in near Kissimmee, FL! (Contract)

Working between 2 LTC facilities (Kissimmee and St Cloud)

Schedule: Monday-Friday (8am-5pm)

Contract: 4-6 month contract

Pay Rate: $65-70/hour

Specific Duties:

• Provide specialized care to high-risk health plan members


• Work as part of an interdisciplinary team


• Conduct comprehensive assessments


• Manage acute and chronic medical and behavioral conditions


• Ensure accurate documentation, including ICD-10 coding


• Communicate and collaborate with the care team


• Facilitate advanced illness and care planning discussions


• Offer counseling and education to patients and caregivers
