As part of the multi-disciplinary team with the National University Health System (NUHS)@Home, you will provide quality pharmaceutical care as our Pharmacist .
Job Responsibilities- Review transfer medications appropriately, contact patients pre-transfer on medication supply needed, and answer medicine related questions.
- Medication counselling on transfer medications by a variety of means (in-person and virtual).
- Review discharge prescriptions appropriately.
- Verify new orders by the physicians and identify inappropriate medication/doses if relevant.
- Assist with performing medication supply duty.
- Follow up on medication preparation status.
- Undertake duties which may be assigned by the NUHS staff member depending on the requirements.
