Conduct psychosocial assessments and provide follow-up (including phone support) for individuals experiencing vulnerability or housing instability. Explain intervention objectives within a structured psychosocial support framework. Facilitate awareness and support activities promoting well-being, social inclusion, and personal development. Collaborate with internal teams and partners within the community network and supportive housing services. Monitor and follow up on individual and group interventions.
