Based on the Continuing Care Case Management Competency Framework you demonstrate ability in the areas of personal and professional accountability, systems thinking, facilitating creative thinking, communicating and building relationships. Using a person‑centered approach, you will develop a care plan supporting the client/family to identify needs, focus on strengths, set achievable personal health and self‑care goals and provide direct nursing care to individuals across the life span.
