The Ponoka Home Care program serves a large area of clients including the community of Ponoka and surrounding areas. We work closely with community groups, other health care services in the community and as part of a multidisciplinary team. Registered Nurse - Case Manager is responsible for directing the care of clients under the home care program. The Case Manager must have strong interpersonal communication and interdisciplinary team player skills, excellent critical thinking, creativity, resourcefulness, time management, and organization abilities.
