Position Overview
We are seeking a compassionate and experienced Registered Nurse (RN) Hospice Case Manager to join a dedicated hospice team serving patients in the Burlington and Walworth communities .
The RN Case Manager will coordinate and manage care for patients with end-stage illnesses , ensuring comfort-focused care in various home-based settings.
