Transitional Care Center (TCC) RN

The Transitional Care Center (TCC) cares for a complex, high-acuity patient population. Patients on this unit require high ventilatory/respiratory support (trach/vents; CPAP/BiPAP) while also having other complex medical needs, resulting in an interdisciplinary care team approach to their care. There is a high emphasis on patient/family education on this unit, as we are transitioning patients from ICU level care to home and families need to learn how to care for their child's complex care needs in the home setting. PRN - 36 hours minimum within a 6-week schedule. Shifts: rotating (day or nights 12hr shifts)
