Description
Position Summary:To provide quality care to clients in the client’s home for crisis and urgent need coverage situations as needed in locations within the four boroughs as specified by the plan of care and in accordance to established policies, procedures.

Principal Responsibilities:
Recognizes and knowledgeable about essential job functions/task with the HHA scope of practice.

Performs and documents service delivery as specified on care plan.

Participates in visit verifications and supervisory/quality field visits as assigned.

Employs principles of Infection Control and Universal Blood and Body Fluid Precautions.

Complies with agency confidentiality policies, and maintains respect for client privacy.
