Support and Service Coordinator - Children's Long Term Support Program (CLTS)

Position Summary: The CLTS Waiver Program is one of Wisconsin's Home and Community Based Services (HCBS) Medicaid Waiver programs, federally authorized under 1915(c) of the Social Security Act. HCBS waiver programs provide Medicaid funding that may be used to fund allowable support and services that promote individuals in the waiver access and inclusion in their home and community. Eligible participants include youth between the ages of 0-22 with developmental, severe emotional, and/or physical needs, who are also functionally and financially eligible for Medicaid. The Support and Service Coordinator (SSC) coordinates and facilitates access to all services and support, both formal and informal, which are needed by the child and family to meet their identified needs.
